I have a good medical plan at work. Why do I need critical illness insurance?
Even quality medical and disability income plans don’t always cover all of your expenses. For example, your medical coverage may have deductibles and copays and may not cover out-of-network treatments. And if you’re out on disability, only a portion of your income may be covered. With the average family spending thousands of dollars during a time of critical illness and recovery,10 most people will need the means to cover extra medical and daily living expenses. Such costs can include deductibles, groceries, housing expenses, car payments and more.
Can I enroll for this coverage without having to take a medical exam?
Yes. Your critical illness coverage is guaranteed issue, which means your acceptance is guaranteed, regardless of your health. You just need to be actively at work for your coverage to be effective. There are no medical exams to take and no health questions to answer.8

How do I file a claim?
File a claim online or with a form by following these steps:
Visit mybenefits.metlife.com or download the MetLife Mobile App to view your certificate of insurance and initiate your claim, or call 866-626-3705 to obtain a claim form.
- Answer some questions about your claim and upload medical documentation and a physician statement to support your claim
- Visit MyBenefits or your MetLife Mobile App frequently to check claim status, letters and benefit payments.
A MetLife claims specialist will review your information, request any additional medical (if necessary) and notify you in writing of a claim decision.
What is needed to file a claim?
To file a Critical Illness claim, you will need to provide authorization for the release of any medical information necessary to process your claim. Your provider will need to provide the following details to support your claim:
- Patient information – name, address, date of birth, gender, and contact information
- Condition information – condition diagnosed and/or treated for, date of diagnosis and first consult for condition, and any
- previous treatment details
- Contact information for any referring and other treating physicians for illness (if applicable)
- Medical provider signature and medical specialty details

Medical information needed is dependent upon diagnosis, however, may consist of itemized invoices/bills, admission and discharge papers from hospital, x-rays, treatment plan details, referral details, death certificate, autopsy, specific tests for diagnosis, hospital and rehab invoices and notes, and/or any other medical information pertinent to the claim.
Please provide any available supporting medical documentation with your claim.
Are payments made directly to me or to my health care provider?
Payments will be made directly to you, not to the doctors, hospitals or other health care providers. You will receive a check, mailed directly to your home. The payment you receive is yours to spend as you see fit and may be used to cover ongoing household bills like:
- Groceries
- Child care
- Mortgage and car payments
- Or any other way you want; the choice is yours
What happens if I have a recurrence?
Your plan pays a Recurrence Benefit for the following covered conditions: Benign Tumor, Invasive and Non-Invasive Cancer, Coronary Artery Bypass Graft, Coma, Heart Attack, Severe Burn and Stroke. A Recurrence Benefit is only available if an Initial Benefit has been paid for the Covered Condition. There is a Benefit Suspension Period between Recurrences.3
If I Leave the Company, Can I Keep My Coverage?
Yes, you can take your coverage with you.10 You will need to continue to pay your premiums to keep your coverage in force. Your coverage will only end if you stop paying your premium or if your employer offers you similar coverage with a different insurance carrier.
