2. Digitized PhilHealth InsuranceID Card
This digitized PhilHealth Insurance ID, aplastic card with a digital code, is optional for employed members. Similar tothe regular type, you can also use this card for faster transactions withPhilHealth.

- This ID card is a valid ID that can be used ingovernment and bank transactions.
- Entitles the owner to discounts on medical servicesand medicines from pharmacies and hospitals that have partnership withPhilHealth.

How to register to PhilHealth
The first step to getting your own PhilHealthID is to register yourself.
For employed people, you may not need to dothis yourself as the first private company you worked in may have already doneyour PhilHealth membership.
If you’re not sure if this was the case, youcan always ask your HR officer. If you’re already registered, then you can askyour HR officer to provide you an ID.
For unemployed and self-employed people whohad no experience as an official employee, you may have to register as avoluntary member and apply for an ID.
To register, visit your nearest PhilHealthLocal Insurance Office and present the following requirements:
- 2x signed copies of the PhilHealth MemberRegistration Form (PMRF)
- 2x latest 1 x 1 ID pictures
- 2x valid ID documents (or a copy of your PSA-issuedbirth certificate)

How to get your PhilHealth ID
Once you have your PIN, you are alreadyregistered with PhilHealth. All you need to do now is to get your PhilHealthID.
For the regular PhilHealth ID
If you have registered on site (in one ofPhilHealth’s offices, the officer should issue your regular card along withyour PIN and Member Data Record (MDR).

- Go to a PhilHealth office (including onesinside malls) and bring your PIN and two valid ID documents.

Currently, this ID type is only available toemployed members. If you’re employed, you can ask your HR officer about this.
Chances are, you need to convince yourcolleagues as well since your HR office might prefer to order this ID card inbulk. Whatever the case, you will have to coordinate with your company’s HRdepartment.
In the past, PhilHealth issues this card toall its members — both employed and voluntary. This option is put on hold forvoluntary members though for some reason.
For senior citizens
There are two primary options for seniorcitizens (with the thought that a helper will do the work).
The first is to the regular path — getting oneat a PhilHealth office:
- Complete two copies of the registration form.
- Bring a 1x1 picture of the senior citizen.
- Show the elder’s issued-ID from the Office ofSenior Citizen Affairs (OSCA) or any valid government ID document.

- Wait for the Member Data Record along with theregular ID card.

Reminders when getting aPhilHealth ID
The recently signed Universal Health Care Lawbasically enrolls every Filipino to the PhilHealth program. However, you willstill need to present the ID card for hospital transactions and for discountsfrom pharmacies and other establishments.
Currently, the digitized card is not availablefor distribution to every member. It’s only available to actively employedmembers. This requires you to update your record in PhilHealth once youremployment status changes.

1. What to do if you lost yourPhilHealth ID?
